Parent or guardian of a child under the age of 18
Children’s Friend, Inc.

A study being conducted by researchers at Children’s Friend, Inc. is seeking to understand the experiences and barriers that families face when accessing mental health services for their children. In order to participate, you must be a parent or guardian caring for a child under the age of 18 with a mental health condition, a medical condition, and/or a rare disease. If you agree to take part in this study, you will be asked questions about different aspects of accessing mental health care. The online survey should take approximately 10 to 20 minutes to complete.

Please be aware that you may find some of the questions sensitive. You do not need to answer questions that make you feel uncomfortable; however, the survey is anonymous. The information gathered from this survey will contribute to a greater understanding of difficulties families face with the hope of increasing families’ access to mental health services.
